Wraparound Care


What is it?

  • A single, integrated Plan of Care, following the National Wraparound Model
  • An individualized plan that is family driven
  • A collaborative team process that supports the family
  • Community based care with family and natural supports
  • Values driven, Needs driven, & Strengths based
  • Grant funded and supported through three major mental health agencies in Yakima County (Catholic Family & Child Service, Central Washington Comprehensive Mental Health, Yakima Valley Farm Worker’s Clinic-Behavioral Health)

Who are we serving?

  • Youth in Yakima County with complex and enduring needs along with their families, ages 10-21
  • Priority given to those involved in juvenile justice or child welfare systems OR youth with co-occurring conditions related to any of the following: substance use, out of home residential placements or psychiatric hospitalizations, school behavior or attendance plans, Special Ed./IEP, placement transition, or others

How does it work?

  • A team of people that are important to the family are brought together to create a plan that focuses on the youth’s strengths and needs
  • An individualized plan for the youth is developed
  • Goals/action steps are implemented and monitored regularly

 

 

 

 

 

How is a youth referred?

  • Referral forms are available electronically at www.yakimacounty.us or www.cfcsyakima.org
  • Completed referrals should be faxed to Yakima County Systems of Care office
  • Referrals will be reviewed twice weekly and referents notified
